Home visitors are here to support parents throughout their child’s early years. Whether the parents have concerns about a child’s development or just want someone to come alongside them as the family grows, there is a program suited for the needs of each family.
The Family C0RE shares one referral form and process.
Home visit programs include:
- Head Start of Yamhill County
- Lutheran Community Services
- Provoking Hope
- Oregon Child Development Coalition
- Grand Ronde Head Start
- Yamhill County Public Health

*Children with chronic medical conditions are eligible up to age 21 years.
If you have children age 0 to 5 years, call 503-455-8050 to connect with our Outreach & Referral Coordinator. You will then receive a customized referral to one of the home visiting programs that make up our Family C0RE.
